TERMINATION FOR NON PAYMENT:
The patient agrees that any treatment they have purchased will be charged on arecurring basis for the 5 week, or otherwise agreed upon, period and agrees to a minimum of a 30 day cancellation notice via email to [email protected] to terminate services and stop recurring payment schedule.
